Lug Nut Problems

its not so much my lug nut..
me and my girlfriend were driving down the road when we heard a knocking, in the front left wheel. so i pulled over and checked it out.. there were only three lug nuts on the wheel, so i figured one of them fell off, but when i tried to put a spare on i realized the whole bolt that the lug nut screws onto sheered right off.. what could have caused that and how do i fix it? please help

take the damaged one of... get a new stud (local parts store or even junkers) and installed the new ones
why did it happen? well that a lot of things but mainly that the bolt is not properly tightened
