Bank1 sensor 1 = precat bank1 sensor 2 = post cat.

When the car comes stock from the factory, it has one on the downpipe between the manifold and the cat, then one after the cat.
You didn't relocate o2-1... You are only monitoring cylinder 4 then? I had to relocate the o2 sensor to get it in the full flow of the exhaust where the 2nd one used to be

Webtech says your p0134 = bank1 sensor 2 inactive. That's the 2nd o2 sensor, after the cat.
